To refer a patient, you may use your own referral form or call our office to speak directly with our referral coordinator. Please include any recent x-rays, CBCT data, or clinical notes via secure digital transfer (contact us for file instructions). If you need a copy of our referral form or would like to discuss a case in advance, our team will be happy to assist.
