Your First Appointment
What to Expect at Your First Visit
We always look forward to meeting new patients. Your first appointment will include a comprehensive surgical evaluation, review of any referrals or x-rays, and a discussion about the best treatment approach for your needs.
Please arrive a few minutes early if you have not already completed your registration paperwork. You may also be asked to complete a health history form and consent forms at the time of your visit.

What to Bring
To ensure a smooth check-in and accurate planning, please bring the following to your first visit.

Insurance Card
Your dental insurance card, if applicable
Referral & X-Rays
A written referral and any x-rays from your general dentist
Medications List
A list of all your current medications
Medical History
Especially conditions like diabetes, high blood pressure, artificial joints, or heart conditions
Parent or Legal Guardian
Required for all patients under 18 years of age
We encourage patients to take all routine medications prior to the appointment unless otherwise advised.
